I understand that massage therapy is not a substitute for medical examination, diagnosis, or treatment. I have disclosed all known medical conditions and medications to my therapist. I understand the risks associated with massage therapy and agree to hold the therapist harmless from any pain, discomfort, or injuries I may experience during or after the session. I understand that I can terminate the session at any time.
